Effective 07/19/2021, and until further notice:

Outpatient and Emergency Department services:

• No visitors will be allowed to accompany a patient into the facility.

• Pediatric patients may be allowed one parent, as deemed appropriate/necessary by the nurse but must pass the screening process prior to entry.

• Immediate family members will be allowed for visitation in end of life situations but must pass the screening process prior to entry.

Inpatient, Observation and Swingbed services:
• Two (2) visitors per patient per day (exceptions apply) will be allowed to enter the facility.

• Each caregiver/visitor must "checkout" at time of departure.

• Immediate family members will be allowed for visitation in end of life situations but must pass the screening process prior to entry.

Exceptions: No visitors are allowed for COVID-19 positive patients, except in the case of end of life circumstances. All visitors entering due to end of life situations are not limited in number but will be required to pass the screening process prior to entry.

• Patient and visitor entry is limited to two (2) entrances. The front entrance will be open with screening, Monday-Friday, 7:30a.m. - 4:00p.m. Anyone entering after 4:00 p.m. or during the weekend will enter thru the Emergency Room-Ambulance Entrance.

• Visiting hours for Inpatients and Swingbed patients are restricted to the hours of 8:00 a.m. - 8:30 p.m., unless circumstances require continued stay from the designated caregiver.

• ALL patients and visitors entering the facility will be screened upon entry and a record of name and contact information will be obtained.

• Patients identified at risk for having COVID-19 will be escorted to a designated isolation waiting area.

• Visitors may be refused entry upon failing screening or if they refuse to follow the facility's policies and procedures.

• Caregivers/Visitors should be instructed to limit their movement within the facility by reducing such things as walking the halls or frequent exiting and entering.

• All visitors and patients are required to wear face coverings. If a visitor or patient arrives without a face covering, a facemask will be provided, as available. Face coverings are not required for children under age 2.

• All visitors will have a designated arm band applied and will be required to wear the arm band during the duration of time in the facility. Failure to wear the armband may result in the visitor being asked to exit the facility.

• Patients being admitted to the facility for observation, inpatient, or swingbed services will be allowed two (2) caregivers/visitors per day to be allowed to enter the building for visitation purposes. The designated visitor must pass the screening process each time they enter the building.

• Caregiver/Visitor must notify staff at the Nurse's Station when they are leaving the facility. The Nurse's Station will notify the screener that the visitor has left the building.
